Project Summary

The Remote Clinic is a health care system that provides services to patients who are not able to visit the doctor at the time of emergency. It is independent of Doctors' location. It works on communicating with Doctors online.  Patients with different concerns can arrive at RHC and can be served with General Physician and Consultant, who will examine the patient and give a prescription. This type of information exchange allows patients to describe their problem and formulate their questions without feeling rushed and there would be a choice for the patient to talk to the Doctor according to his need. the need for RHC is essential for the areas where the people are not provided with basic health facilities and fewer resources to reach to the Clinics or can be served with first aid like Thar.

Several Remote Health clinic booths would be set at different locations (where there is fewer hospitals or clinics unavailability) for the patients. Keeping the sensitivity of health, there would be booths for Doctors as well. The Doctors would be assigned different timings at VCH booth, where he/she can examine patients from different locations online. Most of the booths for Doctors would be set at places where they can easily manage to give time to RHC, like Hospitals. The booths would be monitored by the administration by facial recognition technology while on a video call to the patient.

The RHC would have multiple Certified and Registered doctors connected to it from all over Pakistan. When a doctor comes online to his interface/portal, his availability will start showing on the patient’s interface. The customer can make a call to him if required. The doctor than examine the patient, the doctor then gives a prescription to the patient. For example, there is a patient arrives at the virtual clinic with some eye issues. He can select from the available/online Eyes Specialist from the list of Eye Doctors.

There would be complete Profiles of Doctors (assigned by the administration) with their clear picture, Qualification, Designation, Working Experience, and Ratings according to the patients he/she examines. Several kinds of Doctors/specialists like Physicians, Dermatologists, Urology, Cardiologist, Psychiatrist, Gynecologist, etc. Each category would contain doctors list. The online doctors would be shown available on the RHC interface. Before connecting to a Doctor, the patient would have given the choice of selecting a Doctor from the list of available doctors according to their ratings. And after being served, the patient can rate the Doctor that he has consulted.

For the areas, where people are not aware of using a web-based system or those who cannot brief the Doctor of some medical measurements. Initially, there would be a moderator assigned there. A person who is the moderator at the virtual clinic would have proper nursing education and able to monitor basic health measurements like blood pressure, sugar test, etc.

Project Objectives

The objective of a Remote Health Clinic is to implement a system that helps in improving the health sector particularly in the unprivileged areas of the country reducing the cost of care delivery. The patients can get access to specialists all over Pakistan via this system. The format and delivery of a virtual clinic vary widely depending upon the technology deployed including telephone, online web sessions, and Skype. We will build the website to meet the standards, which will be thoroughly tested against the criteria and will be ready to present by mid-2020.

Project Implementation Method

In the development process of the Remote Health Clinic, Spiral Development Model is chosen. It is the most appropriate SDLC model that includes elements of more process models i.e. Evolutionary, Waterfall and Incremental prototyping. The Design thinking methodology will also be used to understand the requirements of the system to reach its final working prototype.

The development of the interface that provides the services to the moderator to communicate with doctors is a web-based application server that makes it possible. The web interface would be made using bootstrap, WordPress (using HTML, CSS, etc.) the doctors will be assigned with unique User ID and password. When a doctor login the RHC through web he comes online on the moderator’s portal. He can call him if a patient wants to consult the doctor.

The RHC will also contain the records of Doctors, (availability hours, their location, ratings, etc.) Patient record (visiting time, consultant Doctor, prescription, fee, etc.) it will also store the records of the moderator. It will generate a Doctor's salary through availability hours and examining patients. And other records would be maintained by the RHC. To save such records database SQL techniques would be used to retrieve, store the data.

To provide high resolution videoconferencing GPUs (Graphical Processing Unit) would be used to provide the best facilities to the customer. and a clear sound system that will allow the patient to understand the Doctor’s suggestions and quality Microphones would be provided for better communication.

Benefits of the Project

Remote Health Clinic provides ease to the people from traveling for hours to consult their Doctor. Also, the need for VCH is essential for the areas where the people are not provided with basic health facilities and fewer resources to reach the Clinics or can be served with first aid like Thar.

A hypertension module deployed at patient booths to maintain their weekly readings. the system will predict if a patient needs his consult his doctor or not. Also, there would be services provided to patients who don't have enough resources to take their basic reading like blood pressure, sugar test, etc.  

The Sindh Chief Secretary Syed Mumtaz Ali Shah has said that the provincial government is taking all possible steps to resolve the issue of shortage of doctors in Thar. While chairing a high-level meeting on Sunday, Syed Mumtaz Ali Shah said that the Sindh government has decided to take stern action against the doctors who have not joined their duties in Thar. The chief secretary said more allowances will be given to the doctors performing their duties in different health facilities in Tharparkar. (INP, 2018)

If the government take interest in building Remote Health Clinics in Thar instead of forcing Doctors to perform duty and paying high allowances to them. That would be beneficial for both the doctors and the citizens. The doctors won’t be needed to leave their cities to provide health care services to the people. But the RHC will allow the doctors to provide the best services online. The VCH will let the patients (people of the locality, Thar) consult with Pakistan's best Specialist (Doctors) through it.

Technical Details of Final Deliverable

Development of RHCincludes a web-based implemented on WordPress, a database that contains the details implemented on Xampp. Hypertension Module based on machine Learning that Predicts patients of hypertension their stage and if they need to consult Doctors to increase insulin or other medical measures. Facial Recognition system using image processing. Automatic assignment Doctors at different booths according to the choice, a Time table system that will assign duties of Doctors. Integration of sub-modules, testing, and maintenance.
